Sorti de prison après avoir été injustement emprisonné pour une crime qu'il n'a pas commis, Johnny découvre que son propre frère, Sartana, est devenu un bandit et qu'en plus il lui a volé sa femme. C'en est trop pour Johnny qui va chercher à se venger au cours d'une guerre fratricide.
